We recommend that you obtain all of
your forms by downloading (printing) them from this website.
This ensures that you are using the
most up-to-date version of the form which is available.
However, if you do not want to download

Please see our instructions at the bottom of the page regarding
these forms.
To further improve service,
public use forms are being revised and provided in a
format that can be filled out on your computer. In order
to use the fillable features, please note the following:
You will not be able to file
an application via the Internet. Rather, these fillable
forms will enable you to fill out the form, and print
it out - all on your local computer. You must print
out the form and bring/mail it to the school office
in order to file the form.
You will not be able to save this
file on your own computer. We recommend that you fill
out a page of the form, then print it prior to quitting
Acrobat Reader.
If you need the capability to
save information on your computer, you will need to
purchase the full Adobe
Acrobat product.WSR-IV School District does not
require your purchase of these products in order to
use the fillable features of our forms.
In order to make use of all of
the features of our fillable forms, you should use version
5 of the free Adobe Acrobat Reader. You may download
a copy of Acrobat
Reader from Adobe.
